What number should I fax to eBay?

I want to submit my merchant statement to eBay. But I couldn't find the form or fax number in this forum.
Please put this on the top of the merchant section

1. Does eBay still approve merchant account?
2. My ebay account is registered 2 months ago with personal name not a business name, do they require business name?
3. My merchant statement is new, no any transaction on it just one gateway fee. Is it ok?
4. Some friends told me that no buyer want to use merchant checkout, almost all buyers prefer PP. I’m afraid my merchant account is useless.

Most sellers on here that sell a lot seem to use merchant accounts.

In my experience a merchant account is all i have ever used and I made alot of money on a few accounts even with being new and little feedback. So this has not been a problem for me. If your customer wants the product bad enough they will pay however you ask them to

Yes, that number still works,,
You will fax in, and a guy named Wally will call you to verify..
BE CERTAIN--you KNOW the acct info--and u dont get caught off guard on a cold call.

I've already fax my merchant statement, but after 1 month still not call or feedback from ebay. My eBay a/c since 08/2007 and has 2500 points. Not sure if they already decline the mechant application because the new policy.
Please help
