I have the exactly same problem before. I called amazon everyday for about a week until I reached a manager and told her how unreasonable that they asked for statement of a pre-paid gift card. Then she said sorry and unlocked my account immediately.

How did you manage to do that?
The email specifically says that Customer Support is unable to fix the problem, and that you only can send docs to that other email address.
