There aren't any robots. If there are any scripts or software involved it's only in an assisting role, with the ultimate decision being made by a human rep. There's also no "made up criteria" unless you count the reps judgement skills. There was a guy here a few weeks ago (trying to advertise services) who posted screenshots of what the doc review process looks like from Amazon's backend. Every doc you submit is reviewed by a person trained to review docs. They have a checklist they run down to score your docs.
They look for things like discrepancies in font size/type and they've been proven to call companies to verify docs whenever it's possible for them. They've been collecting docs for years so they have something to cross reference against for (I would imagine) any kind of doc you submit to them.
For example, say you submit a doctored Wells Fargo statement that you edited to match your address. You think it's perfect but you actually used the Helvetica font instead of Arial. By cross referencing the doc you submitted with others in their database of submitted docs, that's something that could be detected. And it's something that may be missed by some reps but caught by others.
I think the human review process is why, in general, seemingly identical docs will work sometimes and sometimes they just don't.
