I am not sure how UPS/Fedex does it for sure, but just because you declare it at a higher value does not mean it then has insurance. I believe you need to declare value and insure, goes hand-in-hand.
As the wording suggest, I would think you should only need to pay the extra .85$ but it is something they probably do not regulate, so most payers just pay the extra with never noticing, making them that much more..
Signature just lets you know someone at "that" residence *or the driver* signed for it. But some things like ebay require it for such priced parcels but it would not cover if it is broken. I.e they can still sign for the broken item.
Though Insurance would help more as if it gets losts (never gets signed) you could open a case AND if it is broken when delivered, you could open a case.
