I get this error saying "There was an error with your request. Please try again." with Google Voice when trying to get a new number. How to get around this?

For clarification, what are you using Google Voice for....?

Your forwarding phone number has been banned, presumably for creating too many accounts (I think the limit is 4). Go back to google voice, go to settings, delete the forwarding number, and add a fresh one.
