Is there any possible way I could get it back or is it gone for good? You got hit with the complete laundry list OP - I would wager that the account is pretty dead. If you have money in it, you would likely have to wait 180 days to get it

Ive had the same happen to me (minus SSN as im in UK) answered them all and they still limited it anyway. Id be creating stealth accounts if i were you.
Time to move on to the next account. From my experience when it gets to this state its pretty much game over.
And youre not really walking away from those $600 - youll get them in 180 days (with any luck in 45 days).
