A problem with the Performance ..

i was creating 3 amazon seller accounts, and after finishing the verification step i've received the performance messages as usual asking for the utility bills, so i've sent them, and their details are matched with the business registered address, on 2 of these accounts i've received a message from the performace saying that the've decided that i can't sell on amazon, and for the 3rd account i've received a message from them says that my email isn't associated to an account in the seller central, though i'm sending using contact us button !!!
I've previousely used the same utility bills templates for several accounts and they've worked , sometimes they refuse them and later after some changes they accept them again , but till now i couldn't tell which things they focus on when they review the utility bills , what should i take care of when editing the bill to make sure it will pass ? i'm fixing the names , the barcodes , and even i make sure to know the costs from the service provider to write correct prices ( i don't hide the costs as i'm afraid they would refuse it in such case ) , anything else i should notice ? also should i fold the utility bill printed paper in a specific way to look more real ? and which times are better to submit the bill ?
