If the debit card hasn't been used on another PayPal account and the details match the necessary info on your Paypal account for verification then yes.
I don't think so from what I have read on here - as long as that card was never added to your suspended account. Just don't use your real address when you add it.
But it is not generally recommended to use a debit card with your real info as it could make it easier for paypal to take money from your checking account.

Either.... I am not from the US so what I am saying I got from reading what other people have done.
The name doesn't have to match but you may get caught out at some point, and the card subsequently removed.
I always use my own name on my accounts with variations just to be safe - some banks in the UK will reject electronic transfers if the names don't match.

Things are different in US regarding use of debit cards with PayPal. OP has opened several threads asking the same question. OP needs to read and re-read The Stealth Book xxxxx number of times.. Its there.
