Any European bank lets you create VCC, Some Latin american banks too let you do it and its free of charge I think. At least in most cases.

HX8800.com takes bitcoin but wont let you do those cards...

To withdraw you need to have money on the card. It does not really matter what account you have as long as you get the codes. Even in a country with currency restrictions people have foreign accounts...

Have you tried to call them over VOIP networks?

The only way to not make a VPN visible is basically renting a VPS and then setting up your very own VPN. Need some skills though.
