Amazon Temporarily_Suspended_Account

Hello everyone, and thank you aspkin for your Amazon Ghost Book.

I'm facing a wall, with amazon asking me a billing statement after my first order (buyer fresh-account).
They want :
-Name and billing adresse
-Phone number
-Last 4 digit of my CC
all of these 3 on the same file.

Someone could help me ? What kind of file should I give ?

Thank's for the answer but my account has a different Name and Surname that my credit card.
I also used typo in my amazon address so, amazon address and billing address is a bit different.
Maybe I did something wrong.

You gotta submit info matching your business name/address
