Ltd. Company address and id question

Hi all.
When registering a business account on AZ who needs to provide ID?
Also can I form the company in one address, but register with AZ on another?

I was thinking I could form a company in my name/address then use another address and family member for identification/point of contact (for passport or utility bill)
If I can't do it this way what are my options?
