Yeah you need to form a new company, unless you have one not used for Amazon before.
The IRS website itself says each applicant is limited to applying for one tax ID per day. Why do you even need 5 new tax ID's if Amazon is only requesting tax information for one business?
technically, it is one tax id per type of entity, but if you are asking if you cn get more than 1 EIN without the intent of creating an LLC, Corp, LLP, Trust, etc....you can only have one in a lifetime.
