Name on LLC EIN Form?

Hi guys,

When going through the tax interview for amazon seller accounts, what do you guys put for the name below the business name on the IRS EIN form? Im a bit confused on if my actual name is supposed to be on there. Or is there a way to put a "manager's" name on.

Id like to know this as well for a legit account. My personal account and name were burned long ago so I dont want to put my name in the box. Can I just put the LLC name in here? Does it matter what name gets put in here?
