Has anyone had experience with submitting docs with wrong info to Amazon?

I recently tried to open an Amazon seller account and submitted a bank statement for the initial verification. Right after submitting I realised that I forgot to change the address on the statement!

Do you think there is a chance that I can salvage this account by submitting another statement from another bank (when the verification obviously fails) or is the account toast?

Is the address on the statement linked to another account ?

No it isn't liked to another account.. They just asked me to resubmit the bank statement so I did. Let's see what happens.

as long as the info you submitted wasn't on another shop it should be fine to resubmit a new one

but keep in mind the person reviewing may have the option to view all previous submissions and may deny it simply based on sending the same statement with a different address
