Procedure for reqesting $ from susp. Acct...(Amazon)

I have a 90 day disbursement settlement coming up on a suspended account. Can someone give me the steps, and how to go about contacting Amazon in regard to requesting they disburse my money to my bank account.

Is there a number I call, or should I email payments@amazon, and what do you include in the correspondence?
