question about amazon seller account being suspended
 

for those of you that received failed seller review suspension (without any warning), has anyone used real name and address and have that happen?

Sure - have failed in all kinds of ways ha

i'm wondering why that happends? if using real name and adress, why do we still get failed seller review?

Failed seller review is not only because card/name mismatch. It can happen for variety of reasons like items marked shipped but tracking number is invalid, or too many orders on a brand new account or not enough feedback or A_Z crap.
They will find a reason if they want to.

Lately I receive an email stating that I failed their seller review process so your selling privileges have been removed.

One suspicious thing that has been happened lately is that an order appears and it is not paid for but remains in the pending orders. Also, I receive a message asking me what my return policy is. Has this happened to any of you lately?

pending orders are normal...i've never had anyone ask me about return policy and shortly after getting suspended...
some have said that if you sell tooo fast, you will get failed seller review email without warning.

It happened to me because of
 

Credit card company did not approve the $1.00 authorization on the second month.

Plain and simple.

I have a bunch of accounts and only happened to 1one.
