Is it possible that if we are using a forwarding service for phone verification, such as Evoice or tollfreeforwarding, these numbers are being recycled...and therefore, might be the reason for an account blockage?

Yes - could be recycled - they are overused at this point

The answer is yes.
If you use a number that has previously been used on a suspended account you get your account linked to that one and will probably get suspended with your own account.

Its possible but unless you share other similarities on your account, I don't think its enough to link you and suspend you for a previously used number.

The best alternatives are always real numbers, only then can they be guaranteed to be fresh.
