i got this letter today,
Greetings from Amazon Seller Support,
Thank you for your interest in selling on Amazon.com. As part of your application to sell in the Health and Personal Care category.
I would like to inform you that the documents you have provided is not meeting our minimum criteria.
We accept invoices, purchase orders, sales invoices, or commercial invoices.
We do not accept, retail receipts, packing slips, sales orders, invoices from an online retailer, or pro-forma invoices (Pro-forma invoices are invoices sent in advance of a shipment or delivery of goods. Lack of pricing or shipping information indicates a pro-forma invoice.)
We require additional information. Please submit the required documentation from the following options:
Option #1: Three legible, scanned or clearly photographed vendor invoices or purchase orders for the products you intend to sell in the category. Please ensure the documentation submitted meets our requirements as outlined here:
Invoices must be dated within the last 90 days, or showing item delivery or purchase within the last 90 days
POs must have your name or issuing business name
POs must have contact information for your business (e.g. address, phone number, email or website)
POs must have the names of the products you purchased
POs must have document issuer's name, address, phone number, email or website
Please obscure pricing information (optional)
Option #2: 1 -2 invoices meeting the requirements outlined above plus one of the following items:
Business website URL
Business address of your store/office
Business address of your manufacturing plant
Business certification showing that your business is registered (e.g. business license or sales/use tax license)
Home occupation permit (If you work out of your home)
Option #3: One item from List 1 and one item from List 2

US Food & Drug Administration (FDA), Certificate of Registration
US Food & Drug Administration (FDA), New Dietary Ingredient (NDI) Notification
Acceptable Documents List 2
Business website URL
Business address of your store/office
Business address of your manufacturing plant
Business certification showing that your business is registered (e.g. business license or sales/use tax license)
Home occupation permit (If you work out of your home)
Please attach the required documentation to your email response (e.g., as a PDF file or an image file). We reserve the right to verify all submitted documentation including contacting any product vendors you identify in your application.
If you manufacture or own the brands for the products you intend to sell, as part of the application process, we ask that you submit business certification (e.g., your business license, sales/use tax license, or other registration document) and product testing and safety documentation (e.g., a report from an independent laboratory stating the ingredients in the product, or verifying the safety of the product) or your registration with the US Food and Drug Administration when applicable. We may request additional information on a case-by-case basis. Please note that MSDS (Material safety and data sheets) documents do not meet our requirements for product testing and safety documentation.
