Need help with tax info on stealth account.

I have a Amazon Stealth account and I just got an email that my tax info doesn't match with the IRS and my account will be suspended in 30 days. I used my DBA name EIN and a different address.

I have a EIN assigned to a DBA but on the IRS form CP575 is my personal name and address. Im assuming that is whats wrong. What can I do to get by this? I don't see how to get a EIN without my name on it.
