I've had a PayPal account just been limited and require ID and proof of address. I used my real last name and my middle name as my first name for this account. Does anyone know if they would accept my real ID and proof of address? Or would they refuse it since I used my middle name as my first when making the account.

If this information shows on the ID, i don't see how you'll have a problem. Maybe you prefer using your middle name instead of your first name, not their business to know why, you just need to prove it's you
So it is a real acc and not stealth ? and you already have not been permanently limited with this name and address , if so it wont make problem when the ID verifies the name
