USA EIN Question

I am going to test something for new account creation but it needs to be a multi member LLC business. When I go to register the LLC it only asks for one parties information. How do I get all three members listed on the LLC? I could select partnership but then I dont know if it will still be structured as an LLC. Anyone familiar with this?
