Rep approved return w/o my approval

I just had this stupid _________________________ fill in the blank rep that approved a return that I was calling to get closed. Livid.

She said she did it "accidentally".

They just told me they are going to review the call because there is supposed to be "no way for a rep to approve a return, even accidentally".

Has anyone had this happen, if so what was the end result?
