Possibly - but could be other factors too depending on how you have setup the account and/or how you operate (example: what you sell, your IP source, etc)

Yes, best to use a US bin if paying fees on a US account

Yes. For some reason when I list an item and enter a credit card for the automatic payment method, a ebay suspension follows shortly after. I've never received a suspension when linking PayPal as the automatic payment method.

However, as of late, PayPal will sometimes give an error message when trying to link it to eBay for Automatic Payment method. I think this might be due to PayPal not liking your IP address. So if you if you get more captchas than usual on Paypal it might be a sign there is something they don't like
