Entropay dead?

I tried to withdraw funds from et card to my bank.. It was reversed within one day.. I fill out the form here http://support.entropay.com/customer/portal/emails/new

Have you contact entropay support?

With the issue being so specific, you'd probably get a faster (and more appropriate) response than here.
