Questions about entropay

I have an Entropay account. I have my first card and opened a 2nd virtual card. I noticed 2 things.

1) They don't allow you to put a name on the card.
2) Also they don't support AVS.

Aren't these 2 things needed for PayPal?

I have an Entropay question too.
3) I've read around the forums that it doesn't work in the US anymore. Is this true, anyone?

Entropay is working for the USA.

Worked just last week for a USA account.

Just wanted to make sure. I read something about entropay banned at US online casinos.

I added Entropay CC on eBay and now they are asking for cc stmt with name, billing address and cc# on it. I just contacted Entropay and they said they don't have cc stmt with all that info EB requested. WHAT CAN I DO? PLEASE HELP guys.
