SIM card activation for stealth account

I just bought a new t mobile SIM card to use it for my Stealth account can I use my real information to activate it or use stealth information

if using a sim card for each account would you need a phone that is unlocked and a sim card that is not from a certain company for each account? one more ? about sim card- do they have monthly bill just like a phone or you just pay one time for sims card and thats it?
