w9 form request

I am about to provide paypalwith the requested W9 form.
My question is will irs see the W9 form I fill with paypal.
My problem is I used my middle name as first on my stealth. I got the 1099k last year and it showed as mismatch back to paypal.
my ein was sole proprietor.
But i am scared I might get linked to an old account in my real name if I fill the W9 correctly.
Any suggestions?
