Same **** happened to me, i forgot the address I made and dont have the number its asking for, what i can do its a stealth idc if its limited for 180 days i have 790 in there??

Call them up and try to get a rep that's willing to help you.

Maybe make the excuse that you moved a while back and forgot your old info, but you still have the password to the account. And of course... you should have the email and personal name you put in there
