Submitting EIN- Signing under perjury/address info

When I sign under perjury do I sign under stealth name or real name? And I'm assuming the address needs to be the stealth address on file? Just making sure before I submit my EIN.

I sign under real name since i use my EIN.

With the IRS when I got the EIN I used my real info but I'm asking when you actually go onto Paypal and submit your EIN to them they ask for your info with a clause referring to perjury. This is what I'm asking about.

You sign under your real name when submitting EIN to Paypal? Even though the account is in a stealth name?
