Default Understanding Levels of Account and ID Verification

I am trying to fully understand the different levels of account verification Paypal has. Here is my understanding as of now:

Confirmed - This is the basic start up level, in which you confirm your phone number, email address, etc.

Verified - This is an upper level of verification, used to lift limits on withdrawing / sending money. It is done by completing 2 of the following 3:
1. Adding / Confirming your SSN
2. Adding / Confirming your Bank Account
3. Adding / Confirming your Debit / Credit Card

One more higher level?:
It seems that certain things will make PP request additional information to verify your identity. I believe that they will typically ask for:

-Photocopy of ID (Driver's License, Passport, etc)
-Photocopy of Utility Bill or other Proof of Address
-Photocopy of Proof of SSN (SS Card, Tax Return, etc)

Does this sound right to everyone from their experience?

One more question:
If I have a business account, and do something that triggers PP's request for more information, are they going to want a Driver's License, Proof of Address, and SS Card for the business? (The responsible party, I would suppose?)

Yes they are going to want dl and ss of business owner if you trigger something. I personally never have gotten that trigger yet

So say I had my account (and business) set up in a way that it was an LLC owned by another LLC. Does PP really expect to root all the way down to get the ID & SSN from a holding company business owner?

No but if they don't think you are who you say you are they will request the SS.

Dont sell shady stuff, don't sell values too low for the item and don't get chargbacks/cases and keep your phone active and if your account is 90 days or older, very unlikely you will get the SS request.

I am interested in connection a Paypal Prepaid Debit Mastercard to my account, so that I can instantly load the card from my paypal balance and have instant access to my funds. As I recall when I linked my old card to my old account though, they requested the SS info before they could complete the process of connecting the card to my account.
