What kind of documents will I will need to open an Amazon Associate/Affiliate account?

I saw 3 threads about the Amazon Affiliate program and not a single reply and I'd like to change that.

If you have enough experience could you share what are the key points of developing an Amazon Affiliate website? I'm not asking on how do it, but what are the specifics if your main Amazon Seller account is suspended.
