quick question for biz pp

I've got my S Corp EIN ready, and I want to attach it to 1 of my good running stealth ebay / pp account, however when I click upgrade to business, it tells me to Please provide information for XXXX name , do I just ignore it and just fill out my business address / info ? and when it asks for the last 4 SSN I can just put the last 4 EIN right ?!
