Another decent alternative to Simon is the "all access" visa cards, and they recently started selling them at Safeway stores here in Colorado; both the regular gift card and the re-loadable ones. For me, it's convenient as before I had a hard time making it to the Ace check cashing type places during their business hours to purchase em, now I can grab a card if needed during any one of the 24 hours like when I need Ben and Jerry, or a Bic, etc. really badly at 3, or 4 a.m.
For those that don't know, these "all access" brand GIFT cards (non-reloadable type) must be registered online in order to be used for online or phone transactions, and can be in what ever name and or address you want AND you can change those details at your pleasure as many times as you want (I think!) as long as there is a balance on it via the company website where you can also see past transactions on the card, balances, all the regular stuff....
So you can go to Safeway, pay with cash for a gift card, (don't use your "loyalty club" card for the purchase if you really want anonymity)... then stop by a library or Java hut and register from wherever and with whatever identity you want and you should be stealth...
I have no idea how the reloadable ones work, though, never tried one so the above refers ONLY to the gift card type from this company as first hand experienced..
