If this is a bank issued card, then I believe that the address would have to match the stealth address. You can tell your bank that the address should be on the card for shipping - some merchants will not ship to anything other than the cardholder's address - but banks can add addresses for you. The name can be your real name because Amazon does not see that.
I have not run across Amazon checking public databases the way that eBay does. In my experience, Amazon relies more on their TECHNOLOGY to screen accounts while eBay tends to rely on the old fashioned methods of credit reports and identity verification.
None of this is to say that Amazon CANNOT ask for that, but it rarely happens.
