Due to all this I've basically perfected my method of selling. Whenever I start a new account on eBay I get sales extremely fast. For me, it is hard to just stop making money when it's basically handed to you.
I realize after enough mistakes that volume on newer accounts with managed payments is the biggest factor you face in triggering a review. The only control method you have in stopping it is vacation mode or ending listings. I know some things will sell faster than others, however you still get unexpected sales from time to time.
Personally I think the best approach is to baby the account for about 6 months, and use the holiday season to start growing it stronger. The 90 day window is by far the biggest hurdle on a new account, but even an aged account that hasn't been active is treated the same to a degree. If you can pass at least one holiday season after having used the account for over 6 months with steady sales, chances of a random suspension is low. Assuming you remain disciplined throughout the process.
