This was a long time ago, but when I first got suspended in like 04-06 I had to send a copy of license etc to a place in Utah. I know eBay does have offices in CA, TX and Utah. PayPal I know is in Oklahoma, Arizona and CA for US members.

Sometimes they call you to tell you their going to suspend your account for seller performance.
