how to send you account statement?

hello . im new in this , i was working in ebay with a paypal account then i was banned for a competition and ebay suspended my account , now im trying to return to ebay using a merchant account , i got one in ccnow but i dont know how to send my account statement , do i need to send my id ? or jus the information? please help me with details. thank you
