who uses merchantplus? and are gateways necessary?

So I'm on the merchantplus website. States that Visa/MC fees are 2.15% but I've been reading that they're higher than listed (though those posts are from 2008). What are the Discover/Amex fees?

Also, what exactly is the purpose of a gateway? The merchantplus website states that it has it's own built in gateway that's free.
