What do I submit?

It says US taxpayers only and is asking for the first and last name and address.

How can they be asking for a US taxpayer number if its a Canadian paypal? Something in your story doesn't add up.
If this is indeed the case it must be a glitch or a mistake and you should call them.

Not sure but I provided my info they didn’t ask for anything but my first last name and address. And I got an email saying “Thank you

That's very strange and doesn't make any sense if this is a Canadian account - that 1099 will be useless.

Sounds like a bug in the system. Just ignore them for now.

Unless OP is US resident/US citizen who resides in Canada?
or Canada has its own 1099.
or for some reason [Paypal thinks he is really in US?
