I was looking for it online on the pa website and couldnt find it. This brings a interesting question. So I file the llc using my home address?

I use my home address since that's where all my bih-ness is done. We don't have any brick and mortar stores.

Looks like this is the doc you need:

It sucks that it doesn't look like PA has it set up where you can fill out the forms online. The only part I've ever had to print out and manually fill in is the signature page at the end.
