The ISP must pay to maintain the address space (otherwise it becomes available publicly). There are some small cases where an ISP fails to make payment & can lose the rights to that IP space. This is rare and does not happen to larger companies.
When you see a low number in the first octet of the IP address, this is usually owned by one of the larger companies (or government agencies).
Here is an example:
Comcast owns the 68.53. 0.0/16 subnet range. AT&T cannot assign one of their customers 68.53.3.3 because that is within Comcast's network range.
This is the case worldwide. In order to own a subnet range, a company (or individual) must purchase it so they may manage it privately and use it for its customer base.
Another real world example:
The company I work for (day job) lost one of our address spaces for one of our websites because someone failed to renew the SSL certificate in time. We were able to get it back since no one else reserved it (thankfully).
