Instant Merchant Approval on 2011 Account

Not much info here about adding merchant accounts, if any it seems to be outdated.. My research has told me that pre 11/2007 ebay accounts get an instant merchant account approval and newer may ask for forms.

I have a 2011 account with about 10 recent seller feedback and over 100 total that I just successfully added an authorize dot net gateway to, ran the $1 test transaction and was not asked to fax anything in.

This was my first time adding one so Im not exactly sure what they would ask for besides a merchant statement? My research has told me at least that.

Does anyone else have any experience adding these or with info they may ask?

I plan on getting a new DBA/EIN for each ebay business account.
