LLC Step by Step Question

I downloaded the application for an LLC for my state because I am trying to get my self ready when paypal asks for my ss. The account is going well so I really hate if the account got limited. I have a few question to ask because I don't want to make an error since this is going to cost me $120.00.

Here they are:

1. Filing Party (me or stealth name)?
2. Name of the limited liability company (stealth? correct?)
3. LLC principals office address - required ( me or stealth)?
4. mailing address, if different than above (me or stealth).
