I purchased high limit accounts from here, upgraded them to business accounts and applied EIN's which I got from the IRS, then registered LLC's with my state for each business account. 1099's come...

5% of people bury our heads in the sand and hope it will be ok aslong as we're paying the correct amount of tax anyway(myself included)

Chase is good.

Small local banks and credit unions are ideal.

TD is one to avoid although they are ok most of the time, not always.

You can use whatever address you want. Personally I run 20+ accounts and none of them have an address which is even in my state. If a return is started I either email the buyer our return address...

Nearly all the accounts I've purchased or made myself were done with Gmail accounts. I have one with a hotmail address.

Just about every bank can be used and will also allow you to have multiple checking accounts open, but it's best to spread your banking activity between multiple institutions in case one bank closes...
