This is a good idea. Many buyers simply have no idea that a four in the DSRs can kill your account.
I have a packing slip which explains how misleading eBays wording is, and how badly low stars impact upon small sellers. I also add a small freebie. In the past I have found this to be very successful at keeping up great DSRs. However it doesnt always work.
Recently I had 2 bad buyers who only left 1 star for postage/packing. In both cases they were international transactions.
One buyer in USA didnt read the listing properly and wanted to just pay for UK domestic postage. She sounded so upset I told her "ok I will ship if for this price even though it doesnt cover my costs. The way eBay presents the info on the new listing layout is very misleading. But please bear in mind that I did not design this layout and have no contro over it." She agreed - and then trashed my shipping star.
I only trade in US dollars and 95% of my stuff goes international I agreed to accept a sterming check from a buyer in Cyprus (an expat Brit) and I converted it at that days rates and quoted the item & shipping price. If she thought the shipping was too high that was the chance to query it before she bought. I even send her package before the check cleared so she wouldnt have to wait. She trashed my stars as well.
I wrote to both of them explaining what the impact of their actions had on my business. The US one was a nasty piece of work and did not care but the other buyer was horrified.

When I send the buyer their tracking number I write something similar to this:
Hi. Here is your USPS Express (overnight) tracking number: xxxxxx. Thanks so much for buying from me. Once your bag is delivered, if you are happy with it and my service will you please leave me positive feedback with 5 stars? 5 Stars is most important to ebay. They are suspending sellers for less than 5 stars. I will leave you positive feedback as well! Again, thanks for buying form me and I hope you enjoy your Balenciaga for many years to come!

WAHAHAHAHA... this one nearly crack me up!
Anyway back to the topic... getting my buyers to give me feedback is so BLOODY difficult. What I did is to include a paypal shipping slip where I would wrote "Thank you for shopping with us, please leave us a positive feedback once you are satisfied with your item etc etc etc" but it doesn't seem to be working well." I have shipped out about 40-60 items across all my ebay account and the current feedback I received is only less than 10 for each account. Man... What kind of free gifts you guys add in your postage?
Put a paper with the words *PLEASE LEAVE + FEEDBACK* printed all over it. I mean all over it. Not line after line. But throw it around about 10 times on the paper. Print bulk of them in ink saving format, and include in every order.
